Our team helps the person settle at their own pace, with predictable support and familiar information.
We keep communication open with families and professionals, using learning from day-to-day support to shape the plan.
We focus on the next positive step, whether that is greater independence, a planned move or ongoing specialist support.
Partnership working
Transitions work best when everyone has a clear role and a shared understanding. We work alongside the person, their family or advocates, commissioners, social workers, health professionals and future providers to coordinate each stage of the move.
Multidisciplinary input
Where relevant, we coordinate multidisciplinary input so that transition plans are informed by specialist knowledge as well as the person’s day-to-day experience. This helps identify what needs to be in place for a safe, sustainable move.
Helping translate assessment learning into proactive, practical support strategies.

Transition reporting
We provide clear, practical transition information so the receiving provider and wider network understand what has been learned. Reporting focuses on strengths, communication, support strategies, risks, progress and recommendations for the next stage.
After the move
Our involvement does not end when the move takes place. We offer post-transition support to help the person, receiving provider and wider network embed the plan, respond to early challenges and maintain continuity while the new arrangement settles.
We stay in contact for up to 6 weeks after the move to review how the person is settling and identify any support that may need adjusting.
We support the next team by sharing practical guidance, documentation and reinforce the strategies that helped during assessment and transition.
We work with the network to reflect on progress, address emerging needs and support a stable, positive future.
