Holt House

 

Holt House is a 4 bed short term care service that allows our team to support individuals that fall into crisis, where their previous placement has broken down or is no longer sustainable or safe.

 

Our service model allows for a crisis response, facilitated through Adult Care or Health funding . The individual would then stay within our service for a minimum of 8 weeks, allowing us to support the individual through the crisis, and to work with them on development and goals for their transition our of the service.

 

During their stay, the individual will be supported with a minimum of  1:1 basis by experienced and highly skilled support workers. Additionally, they will work throughout their week with our Nurse and OT, working on goals set the previous week relating to their holistic development. This work will integrate into a final transition report for the family or provider that the individual will transition to.

Our team offer transitional support following the persons stay with us for up to 6 weeks  to enable the long term placement to be successful in supporting the person in the right way to reduce further intervention.

Holt House is located in the heart of the Rochdale community, and has access to local amenities within walking distance. Footprints house is a large and spacious house, decorated throughout to a high standard to create a home environment that is autism friendly.
